How to Register with the Practice
At present we are not able to accept online registrations but hope to change this in the next few months.
We have registration forms for you to complete at the Surgery. It’s helpful if you bring your medical card with you when you come to register.
Patients register with ‘the Practice’ although you will be placed on a doctor’s list. You may ask to see other doctors in the practice at any time although we advise continuity for any one episode of illness.
New patients will be invited to have a general health check when they register with the practice.
Registration Forms
Online Access – for existing patients
In order to register for online access please read the attached form (information leaflet) which explains the terms and conditions of online access. Having read the information leaflet, please complete a registration form which requires returning to the practice along with two forms of identification, one with proof of address and one with a photo, i.e. driving licences, bus pass, passport etc.
If you see any information which you believe to be incorrect in your record, please contact the surgery so that this can be investigated appropriately.
