How to Register with the Practice
You can now register online via the link below or complete the forms by hand, which you can collect from the Surgery.
Patients register with ‘The Practice’ although you will be placed on a doctor’s list. You can ask to see any of the GPs in the practice at any time, although we advise you try and see the same GP for continuity, when being seen for any one episode of illness.
New patients may be invited to have a general health check when they register with the practice, depending on any health issues or if in regular medication. Please speak to a member of staff as soon as possible if you are on regular repeat medication, as you may need to speak to a GP to arrange your prescription.

Online Access to your medical records – for existing patients
In order to register for online access please read the attached form (information leaflet) which explains the terms and conditions of online access. Having read the information leaflet, please complete a registration form which requires returning to the practice along with two forms of identification, one with proof of address and one with a photo, i.e. driving licences, bus pass, passport etc.
If you see any information which you believe to be incorrect in your record, please contact the surgery so that this can be investigated appropriately.
