Mental Health Practitioner Service
The Mental Health Practitioner (registered nurse) is available to provide in-depth mental health assessments, mental health sign-posting or limited short term mental health in-put on a primary care level.
The initial appointments last 1 hour and these take place in a GP Practice setting. Follow up appointments may be offered either on the telephone or in person.
Patients will develop a better understanding and knowledge of their own mental health/wellbeing, and will receive appropriate evidence based, time limited interventions, which includes essential wellbeing skills, such as diet, exercise, mindfulness, anxiety management, sleep hygiene etc.
The Practitioner will also support the Patient to transition into other mental health services i.e. Mental Health Coaches or Community Mental Health Team.
Patients must be 16+ years and consent to the referral.
The Mental Health Practitioner is unable to offer same day appointments. If a Patient is in crisis , needing immediate support , please contact Guideline or First Response.

To access this service, please contact the GP practice and ask to be referred into the primary care mental health team.
Mental Health Coaching Service
This is the first level of support for mental health provision throughout WACA Primary Care Network of GP Practice Surgeries.
Our team of Mental Health Coaches will be based within GP Practices to provide face-to-face, telephone or video call appointments, supporting the mental health needs of patients, working in conjunction with the Social Prescribing teams and also reporting and connecting to other NHS Mental Health Support to provide clinical step up, ensuring patients access the right help at the right time.

Who can access the service?
Patients must be aged 16+ years of age
Experiencing mental health issues and requiring urgent mental health support
The patient must be registered with a GP surgery in the WACA Primary Care Network to access the service.
